4. All services under the Offers must be completed before the expiry of the Promotion Period, save that the first consultation for the Customised Fitness and Health Programme shall be attended by the relevant Cardholder/service recipient on or before 31 January 2025, with the remaining follow-up consultations completed within a four-month timeframe.
5. The services under the Offers are provided by medical practitioner(s) and/or healthcare professional(s) as designated by CUHK Medical Centre and CUHK Medical Clinic. All services under the same programme of the Offers shall be performed at the same Service Venue.
6. Advance booking is required by telephone with CUHK Medical Centre at 3946-6888, CUHK Medical Clinic (Tsim Sha Tsui) at 3468-7141 or CUHK Medical Clinic@Kitchee at 3946-6980 for the relevant services under the Offers. Please notify the staff of the relevant Service Venue which services under the Offers will be used at the time of booking.
7. Availability of the services under the Offers is subject to the service capacities of the respective Service Venues. Upon purchasing or before receiving any services provided under the Offers, Cardholders/service recipients are required to accept the service terms and conditions of the relevant Service Venue. Please enquire the staff of the relevant Service Venue for details.
8. In order to enjoy the Offers, Cardholder/service recipient must present his/her valid Hong Kong Identity Card or passport upon check-in and registration at the relevant Service Venue.
9. The Offers cannot be used in conjunction with other offers or discounts of CUHK Medical Centre and/or CUHK Medical Clinic, and cannot be transferred or exchanged for cash or other goods and/or services.
10. Any fees paid for the services under the Offers are not refundable and cannot be exchanged for other products and/or services provided by CUHK Medical Centre and/or CUHK Medical Clinic.
11. The Offers are only applicable to the Cardholders whose Applicable Credit Card accounts are valid, not in default and in good standing (as determined by the Bank at its sole discretion) during the Promotion Period and when the Offers are issued. If the status of a Cardholder’s Applicable Credit Card account is not in good standing, the Bank reserves the right to disqualify the Cardholder from participating in the Promotion and/or enjoying the Offers.
12. Time and validity of any transactions and/or eligibility of a Cardholder to the Offers shall be determined by the Bank at its sole discretion based on the Bank’s record. If there is any discrepancy between the record of transaction held by a Cardholder and that held by the Bank, the Bank’s record shall be conclusive and binding on the Cardholder.
13. Cardholders must retain the original transaction slips of any transactions. In case of any disputes, the Bank reserves the right to require Cardholders to submit the relevant original transaction slips, other documents or evidence for verification. The submitted transaction slips, documents and/or evidence will not be returned. If there is any discrepancy between the Bank’s record and Cardholder’s record of any transaction, the Bank’s record shall prevail.
14. Participation in the Promotion is subject to there being no abuse/non compliance by the Cardholder, failing which the Bank will debit the values of the Offers from the Cardholder’s account without notice and/or take such action to recover any outstanding amounts.
15. In case of any dispute, the Bank and, as the case may be, CUHK Medical Centre Limited (“CUHKMC”) and/or CUHK Medical Clinic Limited (“CUHK Clinic”)reserve the right of final decision.
16. The Bank and, as the case may be, CUHKMC and/or CUHK Clinic may change these terms and conditions and/or modify or terminate the Promotion.
17. No person other than the relevant Cardholders, the Bank and, as the case may be, CUHKMC and/or CUHK Clinic will have any right under the Contracts (Rights of Third Parties) Ordinance (Chapter 623 of the Laws of Hong Kong) to enforce or enjoy the benefit of any of the provisions of these Terms and Conditions.
18. These Terms and Conditions shall be governed by, and construed in accordance with, the laws of the Hong Kong Special Administrative Region.
19. The English version shall prevail if there is any inconsistency between the English and Chinese versions.
